Adding a Profile Picture

I've been a Premium Spotify member for over 2 years now, and I have never been able to add a profile picture or change the thumbnails on my playlists. It's a little frustrating and I'm just looking for help. The info regarding pictures on the app didn't help, as my desktop app looks different.
