How do I leave a family plan?

I'm currently on a family plan with my ex boyfriend - I've moved out and I want to switch to a single premium account again, but the only way I have been able to find is him doing it as he has the primary account... I'm not in contact with him right now, I just want to get this done. What do I need to do to sort this out without going through him?

Unfortunately, you won't be able to get out of the plan without contacting him. He's the only one who can he remove you.
