I applied a 12 month gift card to my account and it's expired early...

My 12 month gift card, applied Nov 2015, has expired and I've been charged in Sep 2016. Why is this? I had already paid for a month at the end of November 2015 so the gift card should have started at the end of Dec 2015 at the very earliest. It should not expire until the end of this year.
