Shuffle is turned on with no selection or indication

I have a premium account. I notice this mostly on the android app, but see it on the Windows desktop player too. I play an album and do not turn on shuffle, but it still plays the tracks in a shuffled order. When I look at the shuffle toggle, it is off. Usually playing what should be the next song fixes it so tracks play in the correct order, but not always.

Has anyone else seen this? Is there a way to prevent it or a better workaround?
