Is there a way to fill out a form or anything to request to be able to create another development app? 

Im currently using an app on my OBS to show the current song I am playing during recording/stream but i also wanted to add a second app for viewer song requests and after trying to find an alternative, using Songify (which requires it to be a development app) is the only way.

Right now, there does not appear to be a separate form just to request a second Development Mode app.

Spotify's current setup limits Development Mode use quite heavily. A Development Mode app can be used by up to 5 authenticated Spotify users, and the broader route for apps intended for more users is Extended quota mode. That process is separate and has its own eligibility requirements.

Also, keep in mind that the viewer song request setup may need extra care from a policy point of view. If viewers are controlling or influencing playback through a bot during a stream, make sure the app still follows Spotify's Developer Policy around playback, automation, and acceptable use...

I know that is probably not the workaround you were hoping for, but under the current Developer Dashboard limits, creating another Development Mode app may not be something that can be manually requested.
