I do not believe that an app can use mobile data when you disable that for the app.
I have used iOS for years and when it's disabled, like I have done for certain apps, the app can NOT override it.
Spotify already uses data while it is offline or not meant to.. its so they can track what songs you use... it is not a lot but it still happens... read the T&C if you really want....
Many apps have over rode IOS settings by mistake or bugs, they get fixed quickly.
Having 15 years in IT and app area exerience this is something i know inside and out... I am assuming you are guessing based on user experience. In Theory the app shouldnt, but theory is like hindsight... great but not really what actually happens in the real world.
I will wait for Spotifiy to respond.

Well as you know Spotify inside an out... why have you no answer to the above images... what it should do and what it does are different i wish it didnt do it... It never did for the last 3.5 years as a prem user on both ios or android, but it has the last two months since moving to prem family. Spotify can see all the data from their backend, and when they eventually get back to me they will be able to sort it...

LOL you do make me laugh.. do you really think spotify dont look at how you access your music , how it is downloaded... read your T&C ... it is how they get their user data info, which is pretty standard from a BA and marketing point of view.
An IOS app dev will confirm the amount of info you can see is incredible, it may take a long time to get an app accepted but the data you can see and record is incredible.
The two systems talk thats how apps work.
Spotify came back to me last night to advise me this is a known issue on their app and they are working on with Apple, they also credited me the cost of my data charges in free spotify subcription.
The only downside is they couldnt give me a timescale as they hadnt found a solution, setting spotify to not allow cell useage works for some users but not all.
Hope that helps a few people.
How did you get them to credit you? Provide copies of your bill? I've been with Spotify since 2010 and have always been a premium user. I haven't heard anything out of them...
This is great news. How did you contact them? I couldn't find a way to actually contact a representative.
I followed the instructions to reinstall and I think it resolved the issue or at least helped. I listened for an hour last night to all new music and it use 60 MB, which seems more realistic.

Having the same problem with iPhone 7. Reset statistics this morning. Listened to under an hour of downloaded offline content and the Spotify app used 694MB!!! Last month Spotify used over 13gb alone. Absolutely ridiculous considering I only listen to songs downloaded on wifi. Any solutions on this yet?
I deleted the app and reinstalled and so far so good. After a little more than an hour of listening to a downloaded playlist, I've only used 40.5 MB. Last week, it would have been about 400 probably. Tomorrow I'm gonna try a non-downloaded list and see what that does.

How did you manage to use your offline mode? I am trying to use my offline mode but when I switch to offline playback mode and then select a song, it tells me that the song is not able to be played in offline mode. I am confused because I already switched all my playlists to "available online". How do you get your offline mode to work? Let me know please!
