Songs keep skipping or pausing constantly and starts playing when app not open

Recently I have been listening to spotify on my phone and the songs will either pause or skip to the next song repeatedly to the point where I have to stop listening all together because it is so annoying. 

I have twice had it start playing music when I didn't even have Spotify open, today I was on the phone and the call cut out and a song started playing. I had to open Spotify to stop it playing!

Any ideas?
