I've seen multiple profiles of users on Spotify who have followed hundreds of thousands of other users. Linked below are just a few.

How are they doing this? Are they hacking or something like that?

Thanks for coming to the Community.

They likely used the Spotify API to follow lots of users 🙂 I don't recommend doing this though, since it's difficult to unfollow them again (since the API doesn't let you view who you're following).
