Reconnect to facebook?

I accidently clicked 'disconnect from facebook' on my premium account.

When i tried to sign back in it took me to a 'free' account linked to my facebook- but it's a duplicate with no playlists etc. 

I can't remember my username or password for my original account and i haven't received an email to reset my password. 

Still being charged for premium and want to regain access.

Any thoughts???? Thanks!
