Password reset form expired

I've tried to reset my password and have been trying this for 3 months. Every time I go to reset it I get a "sorry,your form has expired. Please try again" this was off of an email that was just sent. Image is attached.

Disconnect from facebook

I deactivated my facebook account and when I went to use spotify it reactivated my FB account. I am a premium customer, I need to change my account setting and connect separate from FB. I will not be using FB anymore.
