spotify premium on iphone - playlist order problems

when I use playlist and it gets to last song, instead or returning to the first song it randomly plays the song in the middle of playlist?  How to fix this?

Also what are playlist icons ?  rectangular shape with arrow in white, green, or with 1 in middle.  This is on right.

It starts playlistradio. You can set playlist to repeat itself after the last song. That should solve your problem. Press that button in the picture.
