When I went to purchase the 99¢ premium for 3 months offer, it said I already used a similar offer. I am assuming this is saying I've used a free trial. I have never used a free trial (I've been waiting until a vacation or something). How can I solve this problem?

Everyone who signs up for Spotify Premium the first time will get a 30-days free trial. That's perhaps why you're not eligible for the offer you are looking for because you may have already used your credit card.
