I used to use Rdio and have switched to Spotify. With Rdio, I could make my playlists on the desktop app, then sync everything over to my iPod with a click. Can this be done with Spotify. 

I wasnt' able to download the latest Spotify app for my iPod because it's an older ipod touch and doesn't go above ios 6

Playlists syncs automatically on your Spotify account and then to even device you have.

Thanks. So how do I add my ipod as a device. I already have the app istalled on the ipod. Do I have to download the desktop app, or can I do everything from online?

For streaming tracks and playlists, you can do it online. To add local files you need to sync them from desktop.
