Spotify Premium has charged me twice over past few months

Spotify Premium has charged me twice over past few months

Spotify - 

 

I have filled out the contact form and received no response.  I have been charged two $12.99 charges on my account since August (I have the receipts from Apple) and need to have these double charges removed because this seems to be a somewhat recent thing.  I don't think I have altered my premium subscription in any way so I need to find out the cause of this and if I can have the second $12.99 charge removed for the past 3 months.
