My Question or Issue

Recently I was unexpectedly logged out of my acct on my phone. When I signed  back in, someone had been listening, saving songs, and creating playlists in my account. This has been going on for a few WEEKS. Everytime I open the app, I am logged out and when I log in the recently played, playlists etc are all changed. I have reset and changed my password multiple times and this does not help. PLEASE HELP!
