Charged full price not special offer price for premium

After the 1 week free trial period I got an offer for 3 months at 0.99. However, I was charged 12.99 per month through itunes. Am I now subscribed twice through the same account and how do I get the $12.99 back?

How do I remove a device?

I've earlier logged into a computer which is not mine, and it is often played music from my account from there. There is no way for me to reach these people. Is there any way to delete that device?
